Liability Waiver & Media Release

By registering for or participating in any program, session, clinic, or camp offered by FalseNin9 Soccer Development, I, as the participating athlete or the parent/legal guardian of the participating athlete (if under 18), acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in soccer activities involves inherent risks, including but not limited to physical contact, falls, collisions, and other hazards that may result in injury. These injuries may include, but are not limited to, sprains, fractures, concussions, paralysis, or, in rare cases, death.

I voluntarily assume all risks associated with participation in any and all activities organized by FalseNin9 Soccer Development, including travel to and from such activities.

2. Release of Liability

In consideration for being allowed to participate, I hereby release, waive, discharge, and agree to hold harmless FalseNin9 Soccer Development, along with its coaches, staff, representatives, contractors, affiliates, and facility owners or lessors, from any and all claims, liabilities, damages, or causes of action arising out of or related to participation in any program or activity.

This release applies to any injury, illness, loss, or damage that may occur during or as a result of participation.

3. Medical Consent

In the event of an injury or medical emergency, I authorize FalseNin9 Soccer Development and its representatives to obtain reasonable medical care for the participant if I cannot be reached. I understand that I am responsible for any costs associated with such care.

4. Media Release

I grant permission to FalseNin9 Soccer Development and its representatives to capture and use photographs, video recordings, or other media (“Images”) of the participant during activities.

I agree that these Images may be used for promotional, marketing, social media, and other lawful purposes without compensation. I understand that all such Images are the property of FalseNin9 Soccer Development.

5. Acknowledgment of Agreement

By registering for services, I acknowledge that I have read, understood, and agree to this Liability Waiver & Media Release. I understand that this agreement is legally binding.
